Decision records (ADRs)

One decision per file, named NNNN-kebab-title.md (zero-padded, sequential). Copy the template from toolkit/templates/adr.md.

Agents read this directory as context and respect accepted decisions — they will not silently reverse one. The merge gate checks that a PR of a flagged change type carries an ADR.
